Along with your application, we must have a copy of the following: Nursing License Driver’s License Signed Social Security Card BLS Card ACLS Card (if applicable) PALS Card (if applicable) TB Skin Test/Chest X-ray (within 12 months) MMR record/Titer record Hep B Series Documentation (or declination signed) Resume GC&SU Background Investigation Questionnaire GDC Criminal/Driver History Consent Form Drug Screen Consent Form Nursing Skills Assessment Upon completion of your application, please call us at (478)453-2222 to make an appointment to bring your paperwork in.
